Spotlight Award Nomination Form

Criteria:

• Nominations are submitted by completing the online form. All nomination information will be listed in the announcement email.
• No self-nomination.
• Nominations may be submitted at any time and will be eligible for the upcoming quarter’s drawing.
• Employees may nominate more than one employee during any given submission period.
• Winners are selected at random from the list of all nominees received during the applicable nomination period.
• All employees, regardless of tenure, can nominate another employee at any other site, company or location.
• All employees, regardless of tenure, may nominate another at any other site or location, however the executive leadership team is ineligible from being nominated / receiving the award. Executive leadership includes: Manuel Zafra Solas, Jim Gill, Julie Drake, Amy Barnes, Chris Teich, and Matthew Fairbanks.
